1. Fill out the medical information form (see below) and submit it to us. Please include good times to call you, keeping in mind that we are in the Seattle area (Pacific time zone).
2. Use the shopping cart to place your order.
3. Receive a confirmation email and an estimated delivery date for your order. NOTE: It may take as few as five minutes or as long as 24 hours for us to receive both forms. Contact us if you do not receive an email after 24 hours - your feedback will help us correct the problem immediately.
4. Jot down your questions and concerns to share during our phone conversation.
5. If you prefer not to receive a phone call, please email us your list of specific questions/concerns that you would like included in the design of your report or resource list.
